Are ANSI standards required just like those for OSHA?

ANSI, unlike OSHA, is voluntary. However, ANSI standards can be used in conjunction/addition to OSHA to provide more complete protection for the worker.

Is it possible to mix and match fall protection equipment such as harnesses, lanyards, etc.?

As long as components are truly compatible-they can be mixed and matched. Be sure to check the OSHA/ANSI reference on the equipment before using. It should be noted that some engineered systems, or specialized equipment, should be checked with the manufacturer of that system as it may not function properly with other devices. There can also be geometrical concerns associated with various connecting devices

What are the different 'fall protection concepts' or 'hierarchy of controls'?

Elimination - Avoiding any unnecessary exposure to a potential fall.
Prevention - using guarding to prevent actual fall exposure.
Arrest - planning the event as if it is going to happen, and minimizing the consequences when it does.
Administrative - monitoring personnel and/or using warning lines.

What is a swing fall and if one occurs is there any shock absorption?

A swing fall occurs if you are moving away from your anchor in a horizontal direction. The same amount of energy is associated with the fall as if you had fallen vertically. However, there is no shock absorption and additionally you risk colliding with some other obstruction that could be near you. This is also known as the pendulum effect.

What is ANSI Z359.0-Z359.14 about?

These go over the definitions of fall protection and fall arrest, safety requirements for fall arrest/fall restraint systems, and rescue system requirements. This was the first installment of the new Fall Protection Code, which is continually being expanded to cover additional topics. Three new sections have already been released.

What is needed to provide a proper fall arrest system?

Three main items are needed: a suitable anchorage, full body support (a full body harness), and connecting means (a lanyard and/or lifeline with shock absorption as needed). Note that these systems typically require the design and installation to be overseen by an OSHA Qualified Person.

What is the definition of fall protection?

This question could be debated. Per ANSI Z359.0 - the definition is "Any equipment, device or system that prevents an accidental fall from elevation or that mitigates the effect of such a fall." This may lack a sense of completeness. Per Dr. J. Nigel Ellis, the definition of fall protection is as follows: "Any effective means of control of a possible injury resulting from foreseeable loss of balance or stability at heights."

What is the difference between a competent person and a qualified person?

A competent person can recognize unsafe working conditions whereas the qualified person has the actual training, experience, certificate or degree to engineer the affected systems and supports. Both of these are able to put a stop to work and correct the problems. OSHA defines these terms slightly differently between General Industry and Construction Industry.

When I am inspecting my equipment, what should I be looking for?

You should be looking to see if there are any edges that are frayed, broken fibers, excessive wear, cuts, broken stitching, any kind of deterioration, discoloration, or cracks. You should always go by what the manufacturer' s instructions indicate. If you feel that the equipment needs repair, the item needs to be removed from service immediately and be checked by a competent person.
